* download claim form from the website.
* Doctor to fill in the form if possible. Otherwise a diagnosis letter. Otherwise, MSIG could contact the hospital.
* Include medical bill
* indicate that ER referred me to orthopaedic specialist
If first bill is clearly presented, MSIG will pay first bill. Subsequent bills can be submitted and paid out later.
